Physical Activity in Migraine and Tension-type Headach

Abstract
Background: Good physical activity for health (GPA) is recommended as an important behavioral therapy for migraine
or tension-type headache (TTH). However, there are few studies regarding the status of GPA and migraine or TTH.
We investigated patient¡¯s perception about influence of exercise on headache, status of GPA, and associated factor
of GPA in patients with migraine and TTH.

Methods: The patients with migraine or TTH with ¡Ã2 headache attacks per month were selected. We defined GPA
as ¡Ã150 min per week of moderate or vigorous physical activity and ¡Ã2 days per week of muscle strengthening
exercise. We assessed patient¡¯s perception about exercise and analyzed the relationship between status of GPA and
the severity of primary headache disorders, psychological status by Hospital Anxiety and Depression Scores, and dizziness during headache or daily life.

Results: A total of 98 patients aged 19-63 years were enrolled: (women 74%): 64 episodic migraineurs, 12 chronic
migraine patients, and 22 TTH patients. Effects of exercise on headache were recorded as deteriorating in 26 patients
(33.3% of episodic migraine, 25% of chronic migraine, 9.1% of TTH, p=0.009). GPA was recorded in 20 patients.
The patients with GPA has lower depression score (5.9¡¾3.77 vs. 8.3¡¾4.15, p=0.02). The proportion of GPA was lower
in women (15.5% vs 34.6%, p=0.04). Monthly headache days, visual analogue scale, and proportion of moderate to
severe impact were not different according to the presence of GPA.

Conclusion: Migraine patients more frequently feel the aggravation of their headache by exercise than TTH. Proportion
of patient with GPA was only one-fifth and female gender or underlying depression was associated lack of GPA.
